Two ways to reach us
Calling is the fastest way to get answers, especially if you are working against a hospital or rehab discharge date. If it is easier to write things down, the form sends the same information to the same advisors.
- By phone at (323) 675-2991. If we cannot pick up, leave a message with a good callback number.
- Through the form on this page. It takes a couple of minutes and asks only what we need to start looking.
What to have ready
None of this is required. It just makes the first conversation more useful, and you can say you are not sure to any of it.
- Who needs care, and what help they need on an ordinary day
- The areas of Los Angeles County that work for family to visit
- A rough monthly budget, and what is paying for it
- The timeline, including any discharge date you are working against
What happens after you reach out
An advisor gets in touch to understand the situation, then goes and does the legwork. You get back a short list of communities that fit, we schedule the tours you want, and we stay involved through move-in. If the answer turns out to be waiting a few months, we will say so.
Our help is free to families because the community pays us a referral fee when a family we introduced moves in. You can read more about how we work and how we are paid.
Where we work
We help families throughout Los Angeles County, including Los Angeles, Pasadena, Glendale, Burbank, Santa Monica, Long Beach, Torrance, Sherman Oaks, and the surrounding communities. We speak English and Spanish.
A note on what we are not
We are a placement and referral service. We are not a medical provider, we do not give medical advice, and we do not operate the communities we introduce you to. If this is an emergency, call 911.
